Analyzing data from three clinical trials with a total of 6,644 patients, they determined that chemotherapy works much better in breast cancer that is estrogen receptor-negative (ER-) than many people think, and conversely, doesn’t work as well in estrogen receptor-positive (ER+) cancer as believed, says the study’s lead author Donald Berry, Ph.D., chair of the Department of Biostatistics and Applied Mathematics at The University of Texas M. D. Anderson Cancer Center.

This conclusion will come as a surprise to many oncologists, Berry says. Women with “node-positive” breast cancer routinely are given chemotherapy, regardless of their tumor type. Women who have ER+ tumors are also given tamoxifen, a drug which inhibits estrogen use by the cancer cells.

“Our analysis shows that tamoxifen works very well for a number of years and taken as a group, there is little or no benefit of even the cumulative effects of modern improvements in chemotherapy for women with ER+ tumors,” he says.

“All in all, this is good news because it shows that the benefit of chemotherapy for ER- tumors is surprisingly dramatic in the same way that tamoxifen’s effect is substantial for ER+ tumors,” Berry says.

The research team, which includes investigators from top cancer centers nationwide, studied outcomes from three large randomized clinical trials which tested the optimal use of chemotherapy in node-positive breast cancer. But none of these trials, all of which were conducted by the Cancer and Leukemia Group B and the U.S. Breast Intergroup, considered estrogen status or whether women had received tamoxifen, largely because the diagnostic importance of estrogen status for chemotherapy was not recognized at the time the trials were designed, Berry says.

Accumulated evidence indicates, however, that improvements in chemotherapy disproportionately benefit women with ER- tumors, Berry says; so the research team decided to statistically model the relative contribution of chemotherapy treatment given estrogen receptor status.

They found the absolute benefits due to chemotherapy were greater for patients with ER- tumors compared to those with ER+ tumors. Specifically, 22.8 percent more ER- patients were disease-free after five years if they received chemotherapy versus 7 percent of ER+ patients. The corresponding improvements for overall survival were 16.7 percent versus 4 percent.

The researchers also compared the different chemotherapy regimens tested in the trials, and found the latest chemotherapy combination studied - biweekly doxorubicin/cyclophosphamide plus paclitaxel - lowered the rate of recurrence and death in ER- patients by more than 50 percent, compared to the low-dose regimen used in the first study.

“This tells us that breast oncology has made enormous strides in treating patients with ER- tumors, a finding which contradicts the prevailing wisdom that with the development of tamoxifen and newer selective estrogen receptor modulator drugs, the benefits of medical science have been primarily focused on ER+ tumors,” Berry says.

“It is true that tamoxifen changed the landscape for ER+ tumors, but the playing field has now been leveled somewhat given the fact that ER- tumors respond well to modern improvements in chemotherapy regimens,” he says.

“We still don’t know enough about autism, but we do know that left unsupported, it can have a devastating impact on those who have the condition and their families. One of the key gaps in our knowledge is simple - we don’t know how many people have the condition in any given area. That is why I am ordering a study to address this. “

The prevalence study will interrogate existing data to record the number of adults with Asperger’s syndrome and high functioning autism. There will also be an additional part to the study on the number of people with autism who have more complex needs and learning disabilities. The research will provide good epidemiological information in terms of prevalence and the characteristics and problems of this group. It will be taken forward by Prof. Brugha at the University of Leicester, in conjunction with a team of research experts including Research Autism and Prof Baron-Cohen of the Autism Research Centre, and will report in 2009.

This work, including research into transitions, is being commissioned and funded jointly with the Department for Children, Schools and Families. An autism expert will be appointed within the Department of Health to take both studies forward.

The National Autistic Society recently called for a prevalence study into the numbers of adults with the condition, as their recent survey found that 63% of adults with autism feel they don’t get enough support. This study will address this problem by informing service providers who can find it hard to plan to meet the needs of adults when they do not have access to good information on the number of people in their area and their needs.

They also evaluated whether the level of antioxidants or seed coat pigments in the bean were related to mammary cancer. The study was funded by a grant from the Beans for Health Alliance, and the Colorado Agricultural Experiment Station with assistance from Archer Daniels Midland Co. and Bush Brothers Inc. Results from the study were published in the January-February 2009 issue of the journal Crop Science.

Cooked dry bean powder from the six market classes and a control group without beans in the diet were fed to laboratory rats in a standard preclinical model for breast cancer. The dry bean powders were also evaluated for antioxidant capacity, phenolic and flavonoid content; all factors thought to be associated with anticancer activity. Chemical analysis of the beans revealed that total phenolic and flavonoid content varied widely among market classes and the differences were strongly associated with seed coat color; where colored beans had ten times or greater phenolic and flavonoid content compared to white beans. Antioxidant capacity of the beans also varied widely among dry bean market classes and were highly related to seed coat color, where colored beans had approximately two to three times greater antioxidant capacity than white beans.

Dry bean consumption from every market class reduced cancer incidence (number of animals with one tumor) and tumor number per animal compared to the control group. Cancer incidence was reduced from 95% in the control group to 67% in animals fed beans. The average number of malignant tumors was also reduced from 3.2 in the control group to 1.4 tumors per animal in the group fed bean. No associations were observed between phenolic content, flavonoid content and antioxidant capacity with cancer among the bean market classes. These results clearly suggest that the anticancer activity in dry bean is not associated with seed color or antioxidant capacity.

Research is ongoing at Colorado State University to investigate the mechanisms and molecules that contribute to the anticancer properties of dry bean. Clinical trials are also underway to determine if bean in the diet of humans are associated with biomarkers for cancer incidence.

After five years of reviewing the latest research findings, a panel of experts led by a Duke University Medical Center anesthesiologist has developed new guidelines to help physicians powder the phenomenon of nausea and vomiting in patients after surgery.

Despite decades of advances in surgical techniques and improved anesthetic agents, one out of three patients still experiences postoperative nausea and vomiting, said Duke anesthesiologist Tong J. Gan, M.D., leader of the panel that developed the guidelines. Such conditions not only can lead to patient dissatisfaction, but also can lengthen hospital stays and prolong recovery, he said.

Gan presented the new guidelines, one set for adults and one for children, on Monday, Oct. 16, at the annual scientific meeting of the American Society of Anesthesiologists, in Chicago. The panel, which included anesthesiologists, surgeons, pharmacists, nurse anesthetists and biostatisticians, was commissioned and supported by the Society of Ambulatory Anesthesia.

The guidelines incorporate the use of new drugs, known as antiemetics, which patients receive prior to surgery to prevent nausea and vomiting. The researchers also found that combining different classes of antiemetics added to their effectiveness.

The guidelines also provide new information that should help physicians identify which patients are at the greatest risk for nausea and vomiting and therefore should receive antiemetics prior to surgery.

“The results of more than 250 trials of antiemetics have been published since the last guidelines were developed five years ago,” Gan said. “The new guidelines incorporate much of this new information and provide physicians with up-to-date strategies for preventing and treating postoperative nausea and vomiting.”

Children are twice as likely as adults to develop postoperative nausea and vomiting, Gan said, and the panel developed a simple four-point scoring system for identifying those children at highest risk. The four factors are: when a surgical procedure lasts more than 30 minutes; when children are three years of age or older; when there is a family history of postoperative nausea and vomiting; and when the surgery is to correct strabismus, or crossed eyes.

“If one of these risk factors is present, then there is a 10 percent chance of postoperative nausea and vomiting,” Gan said. “Each additional factor adds another 20 percent chance, meaning that a child with all four would be at a 70 percent risk. Since the previous guidelines were developed, there have been a number of effective antiemetics approved for use in children, and we would recommend that children identified as being at high risk should be given a combination of these drugs.”

For adults, the factors that elevate risk include being female, being a nonsmoker and having a family history of motion sickness or postoperative nausea and vomiting. Also, long surgeries, or those that use nitrous oxide as an agent of general anesthesia, increase risk, Gan said, as does the use of opioids to control pain after surgery.

The panel concluded that being obese does not increase a patient’s chances of suffering from nausea and vomiting, as was previously thought, Gan said. Additionally, giving patients 100 percent oxygen during surgery — a common preventive strategy — does not appear to be effective in controlling nausea and vomiting.

The research team also looked at nontraditional preventive methods.

“We have seen more support among the medical community for the use of acupuncture before and during surgery to control postoperative nausea and vomiting,” said Gan, who recently conducted a study which found that the ancient Chinese practice is effective for managing the disorder in women undergoing major breast surgery. [link] “However, we also found that hypnosis before surgery was not effective as a preventive measure.”

According to Gan, postoperative nausea and vomiting does not receive enough attention from the medical community.

“There are more than 35 million surgical procedures performed each year in the United States, so this is an extremely important health care issue,” Gan said. “It is also an issue that most physicians do not take seriously enough; they see it as a short-term nuisance that will soon pass. However, studies have shown that nausea and vomiting after surgery is the major factor influencing whether or not patients are satisfied with their surgery.”

The issue is also important for patients who go home in the afternoon after having surgery in the morning.
